Service Description

This consultation is used to interview, asses, and review any movement dysfunctions and lifestyle choices that may be contributing to your pain or functional limitations. A thorough Selective Functional Movement Assessment as well as thorough history will be performed to best determine what exercises, stretches, and lifestyle changes can be made to improve your pain and function. We will review a home exercise program (and you will be emails videos and descriptions) that you can perform throughout your day to help alleviate your pain, improve strength, and change dysfunction. Payment to per performed through PayPal (mikescott.dpt@gmail.com) or Venmo (@mscottdpt620 - Last 4 of Phone 6203). Upon scheduling and payment, intake paperwork will be provided to the requested e-mail and must be returned prior to the start of your evaluation. Cancellation Policy

To cancel or reschedule please reach out to us 24 hours in advance. We have a strict 24 hour cancellation and no show fee of $100.
